PART I: REGISTRATION and PAPERWORK
Register in the Kentucky Department for Public Health Notification System: ReadyOp. Please be sure to enter all contact information including an email address.
You do not have to fill out the sections titled Organization, Tags or Title. Please enter Fayette as your MRC Unit County if you would like to volunteer in Fayette, Bourbon, Jessamine, Scott, Nicholas or Harrison counties.
After entering your contact information at the link above, a registration form will be sent to your email address.
1B. Complete required MRC Paperwork
- The MRC Confidentiality, Code of Conduct, Standard Operating Guidelines Certification and Photo Authorization Form. If you would like to review a copy of the MRC SOGs, they are available for download: 2016 Volunteer Handbook LFMRC
- The background check authorization form. There is no fee required.
- The HIPAA Agreement Form.
- The KYEM50 for Kentucky Emergency Management which ensures workers compensation coverage.
Blank pages are included for printing.
Submit a copy of any medical license or certifications along with a copy of your driver’s license.
PART II: ONLINE TRAINING
Several courses are available online through FEMA, we ask that you review the basic courses and take the quiz at the end. Once you have passed the quiz, you will be emailed a certificate which needs to be forwarded to the MRC coordinator to be placed in your file.
